vCareConnect App helps patients lead a healthy life by helping them follow prescribed tasks by physician or care manager.
NOTE: This patient engagement app is only applicable to patients whose hospital or practice or health insurance company signed up with vcareconnect. Please talk to your healthcare provider or insurance if this app is applicable to you.

Description of features :
- Patients can exchange messages with their friends and family or care manager or physician securely
- Patients can view their care team
- Patients can video chat with their care team ( i.e. friends and family , care manager , physicians)
- Patients request appointments and get reminders on their appointments with physicians
- Patients can report their vitals ( Blood pressure, HBA1C, HDL, LDL etc…) , and see the trends over a period of time
- Patients can read educational materials relevant to their condition , as assigned by the care manager or physician
- Patients can view their medications , add missing medications and mark medications that they have discontinued or not taking any more.
- Patients can update their communication preferences
